The JHC Registered Nurse (RN) provides outpatient nursing care for all patients at the JHC. This position will work as part of a team for an integrated care clinic that combines primary care, dental, behavioral health, and opiate use disorder treatment. The RN will provide clinical and general supervision and consultative services for medical assistants and urinalysis technicians.

As a RN, you will:
  • Provide general nursing care to patients including administration of medications, CLIA waived testing, EKG and treatments in accordance with nursing license and JHC clinic standards
  • Assist providers with clinical procedures
  • Promote a safety-conscious work force
  • Assess, plan, implement and evaluate individual nursing care as clinically appropriate
  • Be responsible for prioritizing and ensuring prompt care for all patient encounters in the order of face-to-face visits, telephone messages and written requests
  • Be responsible for working with the medical staff and medical assistants to ensure efficient patient flow and complete patient care in a timely manner
  • Be responsible for ensuring smooth coordination of care and continuum of care for JHC patients
  • Be responsible for chronic care management of patients who have depression, diabetes, CHF and chronic pain per clinic and practice team policies, procedures, and processes
  • Prepare patients for examinations, screen patients for appropriate information, and instruct patients in collections of samples, specimens, and tests; provide IV therapies
  • Educate and inform patient/family regarding results of laboratory orders and clinical tests, health maintenance, wellness, nutrition, medications, diagnostic procedures and treatments
  • Observe, record and report patients condition including reactions to drugs and treatments to the provider
  • Be responsible for ensuring accurate medical and health information is entered in patients electronic health record
  • Respond to/refer incoming patient treatment related phone calls. Instruct patient and family regarding medications and treatment instructions.
  • Have the ability to cross-train for Dispensing Nurse essential functions and responsibilities
  • Maintain and ensure patient confidentiality
What are the Required Qualifications for this position?
  • Graduation from an accredited school of nursing
  • Valid Washington State license to practice as a Registered Nurse
  • CPR/First Aid Certification or ability to complete employer provided training upon hire
  • Knowledge of professional nursing theory, regulations, and practices to evaluate and provide patient care
  • Understanding of substance use disorders, treatment theories, and medications commonly used to treat substance use disorders such as methadone and buprenorphine

MMU ARNP

We’re now recruiting for several positions for our Mobile Medication Unit (MMU) set to launch this fall. The MMU will be stationed in Forks, WA and will available to those in Clallam and West Jefferson County. Jamestown S’Klallam Tribe has a strong commitment to providing quality healthcare services to our community, and these positions play a crucial role in our mission.

Jamestown Healing Clinic is now recruiting for a MMU ARNP to join the Mobile Medication Unit team! A flexible workweek is required in this role and will be either full-time or part-time, depending on the needs and availability of the candidate.

Position Summary

The Jamestown Healing Clinic (JHC) ARNP will be responsible for caring for patients, maintaining accurate and current medical records, and collaborating with other care providers to provide comprehensive care for patients with substance use disorders. This position will work as part of a mobile unit healthcare team to provide services in rural and underserved communities in Clallam County Washington. This position will provide primary medical care as well as treatment of opiate use disorders with prescription medications such as methadone or buprenorphine to those enrolled at JHC with services being provided in a Mobile Medication Unit (MMU).

Essential Functions
  • Provide primary care to patients of the JHC with appropriate clinical management which may include prescription of medications, referrals, and education/counselling. Manages common health issues that arise including Hepatitis C, Wound Management, URI, HTN, and asthma
  • Diagnose or treat complex, unstable, comorbid, episodic, or emergency conditions in collaboration with other health care providers as necessary
  • Provide medical services including taking a history and physical examination for the opiate treatment program and performs appropriate follow-up visits
  • Accurately and efficiently monitor daily doses of Methadone/Buprenorphine(Suboxone/Subutex)or other medications as indicated by the physician
  • Provide outpatient detoxification and treatment services to persons addicted to opiates
  • Develop treatment plans, based on scientific rationale, standards of care, and professional practice guidelines
  • Provide general health counseling services under the supervision of the Medical Director and follows the progress of patients. Makes appropriate records of findings during examinations and submits charts to the Medical Doctor for review and signature when requested
  • Educate patients regarding how to take prescribed medication and possible side effects
  • Identify complex problems and reviewing related information to develop and evaluate options and implement solutions
  • Cross-train in other aspects of clinic operations as necessary to provide safe, timely, and evidence-based patient care
  • Adhere to all JHC policies and procedures, including but not limited to standards for safety, and work practices as described in the Occupational Exposure Control Plan and HIPAA
  • Other duties as assigned by the Medical Director or Executive Director
What are the Required Qualifications for this position?
  • Licensed MD or DO and in good standing with WA State Medical Board, or able to obtain WA State licensure by February 2022
  • DATA 2000 waivered for the prescription of office-based Buprenorphine.
  • Board certified in Family Medicine, Addiction Medicine, or Internal Medicine.
  • Familiarity with management of MAT prescription medications available (Methadone, Buprenorphine, Naltrexone) for SUDs.
  • Five (5) years’ experience of professional work as a Physician.
  • Previous substance abuse treatment and/or primary care experience.
  • Current knowledge in primary care, mental health, and addiction medicine.

Requirements
  • Graduate of an Advanced Nurse Practitioner program accepted by the State of Washington for licensing; FNP, or AGPCNP
  • Certification by one of the following: American Academy of Nurse Practitioners Certification Board (AANPCB) or American Nurses Credentialing Center (ANCC)
  • Unrestricted license to practice within Washington
  • Current DEA Certificate without restrictions
  • Previous substance abuse treatment and/or primary care experience
  • Knowledge of the current medical practices in primary care, mental health, and addiction medicine. Management of sexually acquired illnesses ,Hep C, family planning and familiarity with prenatal care beneficial

Substance Use Disorder Professional

SUDP will facilitate individual, family, and group counseling for patients participating in the Opioid Treatment Program (OTP).

As a SUD Professional, you will:
  • Provide direct counseling and case management services to assigned patients consisting of individual, group, family; and educational services, including assessment, counseling, and aftercare services
  • Serve as a mediator between patients, relatives, medical staff, and outside agencies
  • Prepare patient progress reports and completion notices and provide to all interfacing agencies to include courts, probation departments, and referral sources as appropriate
  • Ensure compliance and implementation of all JHC policies and treatment procedures; maintain patient confidentiality
  • Effectively orient patients to the facility; provide patients with program guidelines, compliance information including patient rights, grievances, policies, and procedures
  • Identify and respond to crises that may negatively impact treatment and, where possible, uses the negative events to enhance treatment efforts
  • Lead discussions and present lectures and films for group sessions
  • Maintain professional standards and follow the treatment policies established by the JHC, federal, state, and local regulations; maintain professional boundaries with patients and staff
  • Conduct follow-up assessments on treatment programs designed for and undertaken by each patient
  • Coordinate with JHC staff of outside agencies and organizations to expedite treatment for each patient
  • Maintain accurate and current recordkeeping for each patient in accordance with the established electronic records system and all agency procedures and protocols (assessments, treatment plans, treatment procedures, and discharge summaries, etc.)

The Jamestown Healing Clinic (JHC) Dispensing Nurse II-RN will work as part of a team for an integrated care clinic that combines primary care, dental, behavioral health, and opiate use disorder treatment. This position requires administering medications as ordered by medical providers while maintaining accurate accounting of all medication received and dispensed. The Dispensing Nurse II-RN is responsible for safekeeping of all medications and accurate medication record keeping required by the DEA, FDA, and Board of Pharmacy. This is a per-diem position working varied hours and days. Monday-Friday 5:30am-2pm, Saturdays 5:30am-10:30am

As a Dispensing Nurse II - RN, you will:

  • Administer Methadone or Buprenorphine/Naloxone or Monotherapy Buprenorphine (Suboxone/Subutex) as ordered by the Medical Director and staff providers
  • Maintain accurate accounting of all medication received (from supplier) and dispensed (to the patient)
  • Responsible for the safekeeping of all medication stock and the records required by the DEA, State Board of Pharmacy, and JHC
  • Conducts breath alcohol testing, urine collection and processing, reading test results as needed; ensures accurate completion of all forms documenting these procedures
  • Works within scope of license to include measure and record patients’ vital signs, nursing-level physical assessment, patient care or treatment (to include wound care), and patient education
  • Assess patient physical condition, reporting observations to providers either verbally or in writing
  • Alert treatment staff of any observed changes in patient status
  • Enter and update prescriptions and medications in the patients electronic health record
  • Accurately chart patient encounters and patient incidents within the required guidelines
  • Record all inter-agency communications relating to the patient in a timely manner upholding all applicable guidelines
  • Explain the proper use of dispensed medication and its effects to patients
  • Maintain current statistical records
